Key Qualifications

  • Board certified in Family Medicine or Internal Medicine
  • Current DEA and medical license in the State of CA required
  • In good standing with the California State Medical Board and have no significant malpractice judgements or staff disciplinary action in the past
  • Current ACLS certification
  • Minimum of 3-5 years of experience in primary care practice and 2 years of experience working on population health management
  • Proven organizational skills, great interpersonal skills, and ability to work with a wide range of team members
  • Excellent communication skills
  • Ability to think creatively while still integrating evidence-based guidelines and traditional practice patterns when solving problems
  • Strong foundation in guidelines-based and evidence-based medicine with the ability to stay up-to-date with current research
  • Desire and willingness to work in a fast-paced, evolving medical environment

Description

Are you passionate about health promotion and disease prevention? Do you want to improve quality of care with early disease detection and management of chronic diseases? If yes, this role might be for you. As the Population Health Strategist, you are responsible for the development and implementation of evidence-based care management practices in an ambulatory setting. You will work closely with the operations and technology teams, as well as providers, to implement all programs. Additional responsibilities: - Using evidence-based care management practices, identify population health targets (vaccinations, screening programs) and set metrics - Collaborate with a cross-functional team to develop workflows for staff and providers to engage patients on population health to ensure all patients receive recommended routine screenings, vaccinations, and continued chronic disease monitoring - Develop clinical protocols and education to ensure providers are up-to-date with within current guidelines - Develop a strategy and timeline for implementation of population health targets and metrics - Assist in evaluation of incoming population health data and the creation of reports - Collaborate with patient experience team to ensure clinical programs align with desired patient experience

Education & Experience

Medical degree (MD or DO) Holds current DEA and medical license in the State of CA
